ROUTING NUMBER: This number identifies your bank, but it is optional on the endorsement stamp. You can find your bank's routing number on the bottom of your checks. It is the first of three series of numbers separated by symbols. The second number is your check number, and the third is your account number.
ACCOUNT NAME 1: Enter your account name exactly as it appears on your bank account. This can be the name of a person or a business. This line is required.
ACCOUNT NAME 2: If needed, enter a second line of account name information in this field. Line 2 is optional.
ACCOUNT NUMBER: Enter your bank account number in this field. This is required.
